Name, Address, and Party Changes from an NVRA or FPCA Form

Voters who want to change their name or party affiliation can notify the Supervisor using an NVRA or an FPCA form. These forms can also be used to notify the Supervisor of a residential address change.

Note: Voters can also make name, address, and party changes at a DMV office when obtaining/renewing a driver license or by contacting the elections office by signed written notice, phone, or email.

NVRA/FPCA changes can be processed for local voters as well as voters from Florida counties other than your own. Party changes from voters outside your county can be processed by your office, but they should be suspended to the resident county in case a city election in the resident county might require deferral of the voter's party change.

If a voter is moving into your county from another Florida county and their application includes a party change, you can make the address change and party change at the same time. You do not need to save the address change and then reopen the record to make the party change.
