How to
Voters who change their name are required to notify the Supervisor of Elections so that the elections office can issue a voter information card in their new name.
To notify you of a name change, the voter must provide their new name via:
A Florida Voter Registration Application (NVRA form) or a Federal Post Card Application (FPCA form)
Application forms can be submitted to any Florida elections office; they need not be processed by the voter's resident county. FVRS will schedule a new voter information card in the voter's resident county. See Name, Address, and Party Changes from an NVRA or FPCA Form for instructions on processing the form.
Application forms can also be submitted to any Florida driver license office where the change will be processed by HSMV. If the voter’s driver license, Florida ID, or social security number has already been verified, the name change simply flows from HSMV into FVRS and from there to the county database, with no action required on the county’s part. However, if the ID has not been verified, the name change taken by HSMV will come to the county as a suspended application. If a voter's registration status is Incomplete or Pending, you cannot modify their record, but you can make changes to their online application form. Access the application form by going to VOTER REGISTRATION > Application Forms, entering the voter's former name on the Find a Voter dialog; and then clicking Application Search.
A signed written notice containing their date of birth or voter registration number. Written notice of a name change is processed using the Record Correspondence from Voter program.
