Use Text Placeholders in Mail Ballot Request Service Templates

When you customize the text that appears in the Mail Ballot Request Service pages, you can use text placeholders in affidavit and email templates to personalize the information in these forms and emails to include data specific to the current voter's record.

For example, using the [FirstName] [LastName] placeholders will insert the voter's first and last name in place of the placeholder text when it appears on the requestor's page.

Note: If you are using MAIS, you must contact VR Support to make changes to the introductory text.

Enter the text placeholder exactly as shown in the table below in the affidavit, email, or page where you want that information to appear.

This placeholder will be replaced with the:

[FirstName]

Voter's first name on file in FVRS based on the search information provided.

[LastName]

Voter's last name currently on file in FVRS based on the search information provided.

[ResidenceAddress]

Voter's residential address currently on file in FVRS.

[MailingAddress]

Voter's mailing address currently on file in FVRS.

[DateOfBirth]

Voter's date of birth on file in FVRS.

[SelectedDestination]
Important: This placeholder can only be used in the New Address Affidavit Template, the Confirmation Email Template, and Affidavit Is Required Email Template.

Address where the mail ballot will be sent as specified on the Select a Ballot Destination page in the Mail Ballot Request Service. For UOCAVA citizens, if the requestor originally entered a new address, but then later selected a fax or email address as their ballot destination, the ballot destination fax number or email address will display.

[DateRequested]

Date the mail ballot request is being submitted through the Mail Ballot Request Service.

[ElectionName]

Election selected on the Select Election page in the Mail Ballot Request Service.

[ElectionDate]

Date of the election selected on the Select Election page in the Mail Ballot Request Service.

[RequesterFirstName]

First name of the person completing the mail-ballot request for the voter as entered on the Provide Requestor's Information page in the Mail Ballot Request Service.

[RequesterLastName]

Last name of the person completing the mail-ballot request for the voter as entered on the Provide Requestor's Information page in the Mail Ballot Request Service.

[RequesterRelationship]

Relationship of the person completing the mail-ballot request to the voter the request if for as entered on the Provide Requestor's Information page in the Mail Ballot Request Service.

[RequesterAddress]

Address of the person completing the mail-ballot request for the voter as entered on the Provide Requestor's Information page in the Mail Ballot Request Service.
[RequesterSignature] Signature text entered by the person who submitted the mail-ballot request.
[Last4SSN]

Last four digits of the voter's Social Security Number that, when provided, is included in the Affidavit form.

[DriversLicenseNumber]

The driver's license or state ID number that, when provided, is included in the Affidavit form.
